To exit the survey you first need to exit Kiosk Mode, to do this first select "settings" in the lower left corner

After selecting "settings" a pin screen will appear.

Please enter your pin, if you don't know your pin, please contact us at support@pulseforgood.com.

After entering the pin, tap the "Kiosk Mode" slider to off.

Survey can now be exited by pressing the home button.

Note: Kiosk mode re-enables after a few minutes if you don't exit the app, it also re-enables when the app is re-opened.
